In the drop down menu above you will find Conroe ISD and UIL forms for students. We want to make sure that we have as much information as possible to take the best care of your student athlete. These forms help us to better understand a students condition, inform us of emergency contact information, dosing instructions for medications, self carry/administration and emergency care plans.

If your student has asthma, a severe allergy, diabetes, or a seizure disorder they are REQUIRED to have the proper Conroe ISD health forms on file to participate in athletics at Conroe High School. Please note that all forms must be signed by a physician to be considered complete.

Note: If you have a student at Conroe High School 9th grade campus that goes to the main campus for athletics he/she will need medications on both campuses unless he/she has taken the steps necessary to self carry medication (asthma & severe allergy).
